The fourth stage of the Tirreno-Adriatico takes the peloton from Tagliacozzo to Martinsicuro for 213 km and is the third consecutive day exceeding 200 kilometers. A stage that is certainly not straightforward, also because it immediately presents two classic Apennine climbs such as Ovindoli (km 36) and the Valico delle Capannelle (km 100.2).

From the categorized climb begins the long descent towards Teramo and from here the peloton enters the final part of the stage which features a sequence of walls and "small walls". Riders will climb Castellalto (gradients around 12% for long stretches, km 164.3), Mosciano Sant'Angelo and 12 km from the finish the last categorized climb of the day, the Tortoreto wall via Badetta, which reaches 20% in the final section. Following this is a short descent before the 8 km along the seafront that lead to the finish line.
