UCI AGAINST CO REBREATHERS TO TEAMS «DO NOT USE THEM», TO WADA «BAN THEM»

There are new modifications to UCI regulations and this time the International Cycling Union is targeting CO rebreathers, that is, carbon monoxide. During the UCI's annual seminar in Nice, the International Cycling Union launched an appeal to riders and teams to no longer use carbon monoxide rebreathers, which are reportedly being used particularly by Visma – Lease a Bike and UAE Emirates.
CO rebreathers are currently used to measure hemoglobin mass in the blood. A limited amount of CO (carbon monoxide) is inhaled and then binds to hemoglobin, the blood protein that carries oxygen. The amount of CO absorbed comes at the expense of oxygen, so we can see through a finger prick how much hemoglobin is saturated with CO. This way, it is possible to ultimately determine the total hemoglobin mass. It should be noted that this system originated among miners and divers.
This little-known system is a very effective method for evaluating whether a high-altitude training period has had an effect, but if this was the initial idea, today this technique is also used as an alternative to high-altitude training. Consequently, using this method, there will be less oxygen in the blood, a deficiency to which the body will respond. According to a study conducted at the University of Ghent, this would increase the natural production of EPO and hematocrit. Moreover, considering that carbon monoxide remains bound to hemoglobin for four or five hours, a lower blood oxygen saturation would be obtained for a long time. In simple terms, if CO is inhaled four or five times a day, it will have the same effect as spending an entire day at high altitude. After some considerations, it is reasonable to think that the CO rebreather could be considered a doping system, but currently, this is not the case.
Even though the UCI has asked the World Anti-Doping Agency WADA to take a position on this matter, this methodology - at least until now - has not been entered in the anti-doping book. Furthermore, it should be said that the available data are still few, so it is not possible to establish how much this technique could help the athlete, but at the same time nor how much it could harm them. In conclusion, there is an alert and a request not to use the CO rebreather, and the data that will be collected will serve to open a study that can provide important information on effectiveness, side effects, and any advantages that would benefit the rider using it.
